Custom Professional Mouthguard FAQ

In the brutal arena of combat sports, every piece of gear is a shield. Your hands are wrapped, your shins are guarded, but what about the most vulnerable weapon you possess: your jaw? For MMA fighters, boxers, and other high-impact athletes, dental trauma is one of the most common and costly injuries. A wired jaw costs more than a proper guard. This isn’t about comfort; it’s about staying in the fight, avoiding unnecessary downtime, and protecting your ability to compete.

A dentist-designed custom mouthguard for MMA is not a luxury; it’s a non-negotiable piece of professional, dental-grade protective equipment. It’s the difference between walking out of the cage intact and facing months of recovery, dental surgery, and lost fight opportunities. Unlike boil-and-bite guards, professionally made guards are engineered with impact management in mind, designed to help distribute forces across the jaw rather than concentrating them in one area, with TMJ alignment and occlusion considered as part of the design process.

We’re talking about precision-engineered, dentist-designed mouthguards, not boil-and-bite compromises. Impact Dental Designs builds every high-impact mouthguard using detailed dental impressions, occlusion analysis, and functional TMJ considerations, delivering a secure fit that’s harder to dislodge during heavy breathing or a hard strike. That level of retention and stability is why serious athletes move beyond over-the-counter guards.

This guide cuts through the noise by focusing on what actually matters in fight-grade protection. We’ll lay out why a custom mouthguard designed by Dr. Andrew Sarowitz—former team dentist for the Brooklyn Nets and New York Liberty and a trusted provider for athletes competing in the UFC, PFL, Bellator, and ONE Championship—is a go-to choice for fighters who take their craft seriously. Understand the engineering behind impact absorption, the role of proper jaw alignment, and why elite fighters rely on custom mouthguards built specifically for combat sports, so you can focus solely on the fight — not on your teeth.

Are these the same mouthguards the UFC fighters wear?

Absolutely! They're built to the exact specifications as the professionals.

How long does it take to make a guard?

The normal turnaround time is about 5 weeks but that can fluctuate based on the complexity of the design and how quickly the molds are returned. If you have a specific date you need your mouthguard by please send us a message.

Why is the shipping cost so expensive?

All first time purchases have to be triple shipped which adds to the total cost. A molding kit is shipped to you with an included pre-paid return label. Then once we complete your guard it's shipped back to you.

How long do the mouthguards last?

Depending on the frequency of use the typical shelf life can approach two years. An exception would be in kids/teenagers who are still growing. In those cases the guards would likely need to be replaced after one year or sports season. Another exception would be combat sports athletes who use their guard for every training session. We recommend reserving the mouthguard for sparring and competition. If you use it more selectively it can last for two years or more.

How do when I know it’s time to replace my mouthguard?

The most important sign to look for is reduced retention. What that means is that the guard will fall out more easily when your mouth is opened. Another sign would be any kind of separation of the layers of the guard. That can happen after repeated biting/clenching.

Can I get a guard made if I have braces?

You can get a mouthguard made with braces Yes! But, the molding kits won't work. You would need to get an intraoral scan digitally and have that sent to our lab. If you're local we can do that here in New York but if you're not you can have that done at any local dental office. We can also sometimes offer a courtesy discount. If you're considering  purchasing a guard for an athlete with braces shoot us a message and we can help coordinate.

Can I use my guard after I’ve had dental work done?

The short answer is yes. For most routine and minor dental work the mouthguard should still fit. However for major work it may have an effect. We would consider major work to be; ortho/braces, teeth extractions or removal, crown and bridge work, dental implants. Things like fillings should not cause any issues.

Why is a custom professional mouthguard superior to over-the-counter options for fighters?

A custom professional mouthguard offers a precision fit designed around your occlusion and jaw mechanics, impact management built for high-contact sports, and a low-profile design that supports controlled breathing and communication, making it the preferred option over generic boil-and-bite guards for serious fighters. Generic guards provide a loose, bulky fit that compromises protection and performance, often shifting during impact.

Custom mouthguards from Impact Dental Designs are dentist-designed and fabricated from detailed dental impressions, ensuring a secure, stable fit intended to stay in place during training and competition, with design features focused on consistent positioning and impact handling rather than one-size-fits-all padding. This level of clinical design prioritizes retention, fit, and durability for combat sports use, so poorly fitting gear isn’t what you’re thinking about mid-round.

Impact Dental Designs mouthguards are built specifically for high-impact combat sports—not general fitness or recreational use.

Takeaway: Serious fighters choose a dentist-designed custom mouthguard built for MMA and other combat sports because fit, retention, and fight-grade construction matter more than convenience.

What specific dental injuries does a custom mouthguard prevent in combat sports?

A custom professional mouthguard is designed to help reduce the risk of common dental and oral injuries seen during training, sparring, and competition in combat sports. It is engineered with impact management in mind, helping protect against chipped, fractured, or displaced teeth during strikes. The guard also acts as a barrier between the teeth and soft tissues, helping limit cuts to the lips, cheeks, and tongue caused by sudden contact. By maintaining consistent positioning between the upper and lower teeth, a custom guard is designed to stay seated during impact without interfering with natural movement.

Takeaway: A custom mouthguard built for MMA and other combat sports helps reduce dental damage and soft tissue injury by prioritizing fit, retention, and impact management during training and competition.

How does a custom fit impact a fighter's performance and safety?

A custom fit is designed to help reduce gear-related distractions and support a fighter’s safety by ensuring secure retention, consistent positioning, and a design that does not interfere with natural breathing during training and competition. A guard that fits perfectly stays in place during intense action, eliminating distractions and the need to clench or readjust. This secure fit focuses on consistent positioning rather than forcing the jaw into a fixed hold, helping fighters avoid unnecessary tension while moving and striking. Plus, the precise molding allows for clear speaking, enabling fighters to issue commands or communicate with their corner without removing or repositioning the guard, keeping exchanges controlled and predictable.

Impact Dental Designs mouthguards are dentist-designed by Dr. Andrew Sarowitz, drawing on over 15 years of clinical experience, and are built using detailed dental impressions, occlusion analysis, and TMJ-aware engineering to meet the demands of high-impact combat sports.

Takeaway: Built to support performance and safety, a custom mouthguard for MMA prioritizes retention, fit, and reliable communication during training and competition.

What materials are used in custom professional mouthguards for maximum protection?

Custom professional mouthguards are crafted from high-grade, multi-layered dental thermoplastics selected for use in repeated high-impact contact environments, often incorporating different material densities for durability and structural support under force. These dental-grade polymers, commonly including ethyl vinyl acetate (EVA), are chosen for their durability, ability to hold shape under repeated strikes, and suitability for intraoral use. Some designs integrate a firmer outer layer to handle direct contact and a more adaptive inner layer designed to conform closely to the teeth and bite. This strategic layering is engineered with force distribution in mind, supporting fit, retention, and longevity in combat sports environments.

Takeaway: Benefit from multi-layered, dental-grade thermoplastic materials used in custom mouthguards built for MMA and other combat sports, where durability, fit, and consistency matter during training and competition.

How does the design of a custom mouthguard absorb and distribute impact forces?

The design of a custom mouthguard is built to handle repeated strikes and contact through its dentist-designed, vacuum-sealed fit and multi-layered material construction. When an impact occurs, the custom-molded guard is shaped to sit securely around each tooth, helping limit direct tooth-to-impact contact during strikes. The firmer outer layers are designed to take the initial contact, while more adaptive inner layers conform to the dental arch, with force distribution considered as part of the overall design. This approach prioritizes consistent positioning, fit, and retention during high-impact contact, supporting use in combat sports environments.

Takeaway: A custom mouthguard for MMA is built with precise fit and layered construction to handle repeated impact during training and competition.
